What the inspector wrote

On Wednesday, 26 March, 2025, at approximately 10:00 am, Licensing Program Analyst (LPA) Fabian Schwartz met with Licensee's Assistant, Jahne Abarca, for the purpose of an unannounced Plan Of Corrections (POC) inspection. Upon arrival, LPA observed licensee's assistant and licensee's spouse supervising 10 preschool children, 2 of which were infants. All individuals subject to criminal background review have obtained a criminal record clearance. Facility hours of operation are 24/7. Licensee was not present at facility during time of inspection and Assistants informed LPA that Licensee would be gone all day. On Tuesday, 17 December, 2024, LPA conducted an unannounced annual inspection to facility. During that inspection, Facility was Cited 3 Type A citations and 5 Type B citations. Type A citations were for facility having poisons at facility which were not stored behind a lock, having chemicals accessible in two places in facility, and not having a full size 2A-10-BC fire extinguisher at facility. Type B Citations were for not having stairs barricaded at facility, not having a current mandated reporter certificate, not having a current roster at facility, facility not maintaining sleep logs for infants in care, and not maintaining a fire drill log with documented fire drills at least every 6 months. During today's inspection. LPA observed Chemicals and Po isons being stored properly, a full 2A-10-BC fire extinguisher in facility, a current roster, a fire drill log dated 19 December 2024 , and stairs at facility being appropriately barricaded. During today's inspection, LPA has cleared all of the corrections listed above. Deficiencies which still need correction are Assistant's Mandated Reporter Certificate, and Sleep logs for infants in care which Licensee stated they would send to LPA by end of week. Report Continued on LIC-809-C.......... Report Continued from LIC-809......... A notice of site visit was given and must remain posted for 30 days. Based on the inspection, No Title 22 Deficiency citations have been issued, but facility will receive a Technical Violation for having Licensee absent from facility for longer than 20% of the operating day - Title 22 Regulation 102417 (a) . The Technical Violation is Explained further on accompanying LIC 9102 form. Exit interview conducted and report was reviewed with the licensee's Assistant Jahne Abarca. During the exit interview, the Licensee's Assistant confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
