What the inspector wrote

Licensing Program Analysts (LPAs) Loraine Perez and Andrea Cortez met with Licensee Jessica Alvarez, for the purpose of conducting an unannounced case management- deficiencies inspection. Present today is Licensee and her adult daughter who is also an assistant. All individuals subject to criminal background review have obtained a criminal record clearance. Inspection was conducted in Spanish. Facilities hours of operation are Monday through Friday 5:00 AM to 6:00 PM. During today's inspection, LPAs toured the facility, conducted interviews, observed care, and obtained relevant documentation. At the time LPA's arrived there were three infants, and five preschools for a total of eight children being supervised and cared for by the licensee only. Licensee's assistant arrived to the facility at approximately 1:20PM. There were no school age children present at the time LPAs arrived. From interview, Licensee was alone for approximately 20 minutes. Licensee did not maintain ratio of a large license, deficiency is cited and recorded on the LIC809D. LPAs observed loose objects in the portable play crib with a sleeping infant. A deficiency is cited and recorded on the LIC809D. LPAs observed an infant 1.5 years old, with no food or milk/water while in a highchair watching T.V. for approximately 15 minutes during inspection. LPA Cortez brought this to the attention of the assistant and child was removed from the highchair. A deficiency is cited and recorded on the LIC809D. Report continues on LIC809C. LPA Loraine Perez informed licensee Jessica Alvarez that this report dated 02/05/2026 documents three Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, or personal rights of children in care. Also, LPA Perez informed the licensee to provide a copy of this licensing report dated 02/05/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Notice of site visit was given and must remain posted for 30 days . Exit interview conducted and report was reviewed with the licensee Jessica Alvarez
