On Friday, October 10, 2025, Licensing Program Analyst (LPA) Lea Habtom arrived at Vineland Preschool for an unannouced case management inspection. There were 8 preschool children being supervised by 4 staff. There was 1 contracted personnel providing direct services to a child from a third party agency. All staff present today have fingerprint clearances and associations. LPA met with director, Kabao Vang, and notified her the purpose of the inspection is to follow up on the self reported unusual incident. The facility reported on October 2, 2025, a child (C1) was found outside standing by the classroom front door by a staff member (S1) after returning from lunch. The child was brought back into the classroom by the staff member (S1). The child was was outside unsupervised for about 2-3 minutes before being brought back into the classroom. LPA conducted staff interviews regarding the incident. Following the incident, the program, installed a ring bell inside the classroom that rings when the door is pushed open from the inside of the classroom. A ring bell was also installed on the gate leading to the parking lot that alerts staff in the classroom once the door is opened. Staff have also made adjustments to stay in one classroom for whole day. The classroom size can accommodate the number of kids currently enrolled in the program. LPA notified the director to reach out to the fire department to seek recommendations on items that can be used, if any, by the door without blocking the door access. Deficiency cited on 809-D. LPA Lea Habtom informed the director Kabao Vang that this report dated October 10. 2025, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the director Kabao Vang to provide a copy of this licensing report dated October 10, 2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
