What the inspector wrote

Licensing Program Analyst (LPA) Corina Beckby met with Program Lead, Song Her to follow up on the Unusual Incident Report (UIR) emailed to Community Care Licensing on April 02, 2026. The facility reported the UIR to Community Care Licensing within 24hrs and submitted a written UIR within 7 days. The center self-reported that on 03/30/2026, at approximately 4:43 pm, a 4-year-old student leaped for a monkey bar and fell resulting in a bone fracture. Field Staff stated there weren’t enough wood chips underneath the bars. LPA observed little to no wood chips where the child fell and saw the exposed cushion padding underneath the wood chips. LPA also observed a faded sticker on the playground equipment with the number “5” indicating the youngest age for a child to use the equipment, per manufacturer. Based on the interview and information obtained, a Title 22 Deficiency has been issued on the attached LIC 809-D. LPA Beckby informed Program Lead, Song Her that this report dated 04/20/2026 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Beckby informed Program Lead, to provide a copy of this licensing report dated 04/20/2026 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Facility has been provided with appeal rights. Facility evaluation report was reviewed and discussed with Program Lead, Song Her. Exit interview was conducted. A Notice of Site Visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

    Personal Rights- (a) The licensee shall ensure that each child is accorded the following personal rights: (2) ...safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.This was not met by: Allowing a 4-yr-old on the school playground structure not equipped for the childs age and not having enough cushioning underneath the monkey bar area.
