What the inspector wrote

On 07/18/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a Case Management inspection to verify corrections of a deficiency cited on 05/14/2025. LPA arrived at the Family Child Care Home (FCCH) and was met by Licensee Maria Medina Reveles (L1). LPA disclosed the purpose of the inspection and was granted entrance into the FCCH. LPA toured the facility and observed 1 infant and 7 preschool aged children being supervised by L1 and an Adult Aide. LPA determined, through accessing Guardian, that all required adults were background cleared and associated to the license. On 05/14/2025, the facility was cited a Type B deficiency for allowing children in the OFF-limits back yard of Building B. Licensee has since made changes to the backyard to include mesh on the surrounding fence and requested that the back yard of Building B be on limits. LPA toured Building B and observed that hazardous items were inaccessible to children in care. LPA inspected the backyard of Building B which includes a play structure. LPA observed wood chips under the play structure to provide cushioning. LPA told L1 to keep the manufacturers information in her facility files and to abide by the manufacturer's recommended age range of 2 to 12 years old. LPA and L1 walked the property and ensured that all buildings and structures were represented on the updated Facility Sketches and were clearly labeled "off-limits." LPA inspected the spa and observed a locking cover that could not be lifted. LPA reminded L1 that OFF-limits areas must remain inaccessible to children in care. The Type B deficiency was cleared during today's inspection, and LPA provided L1 with Proof of Correction letter. As of today, 07/18/2025, the backyard playground of Building B is on limits. No deficiencies were cited during today's inspection on 07/18/2025 in the areas that were evaluated. An exit interview was conducted, and the report was reviewed with Licensee Medina Reveles. Licensee Appeal Rights were provided to L1 by LPA. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100.
