What Happened
Complaint investigation substantiated serious allegations of physical abuse and inadequate diaper care. Children reported being hit, slapped, grabbed forcefully, and thrown. Multiple children arrived with wet/soiled diapers causing rashes. Two Type A deficiencies issued.
Observations
- Child demonstrated to parent how licensee grabbed, spun, and threw them to wall
- Children reported being hit on forehead, slapped on neck and face, grabbed with tight grips
- Assistant reported giving bear hugs and restraining rather than consoling children
- Multiple children presented with wet/soggy diapers at pickup
- Children presented with diaper rashes and dry poop on private areas
- Assistant only changed one child's diaper; licensees responsible for rest
- Licensees left facility for 4 hours; two female children not diapered during that time

This requirement was not met by: Licensees did not change diapers in a timely manner ressulting in diaper rashes, and/or did not properly clean excriment off children's private areas
