What the inspector wrote

Licensing Program Analyst (LPA) Loraine Perez met with Licensee Elshebia Ekekwe, for an unannounced case management deficiencies inspection . Licensee and two assistants were present providing care and supervision to three infants, three preschool and one school age children for a total of seven children. LPA toured the facility and observed care at the time of inspection. It was observed that two infants were napping in rooms separate from the room the Licensee and assistants were in. The door to the rooms were not maintained open while the infants were napping. Deficiency is cited and recorded on the LIC809D. Licensee has a monitoring device in each room and was not aware of the regulation requiring the door to the room the infants are sleeping in shall remain open at all times. Licensee reviewed regulation with assistants at the time of inspection, deficiency is cleared, bringing this facility back into compliance. LPA Loraine Perez informed licensee Elshebia Ekekwe that this report dated 05/19/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Loraine Perez informed the licensee to provide a copy of this licensing report dated 05/19/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Elshebia Ekekwe .
