What the inspector wrote

Licensing Program Analysts (LPAs) Loraine Perez and Dao Vang met with Licensee Elshebia Ekekwe, for the purpose of conducting an unannounced subsequent complaint investigation inspection pertaining to the above allegation, provider left child in soiled clothing. The purpose of today's inspection was explained to Licensee There is a census of eight children being cared for and supervised by Licensee and one assistant. During today's inspection, LPA conducted interviews, and observed care. Witness statements, and LPA observations failed to corroborate the allegation, provider left child in soiled clothing. Witness statements reviled accounts of children sent home in clean clothing and diapers, and some children sent home in dirty diapers and clothing. LPA observed children clean and observed diaper changes done by the assistants. Although the allegation may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Licensee Elshebia Ekekwe. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.

    The provider shall supervise infants... : If the infant is sleeping in a separate room from where the provider is stationed, the door to the room the infant is sleeping in shall remain open at all times.This requirement is not met as evidenced by: Based on observation the licensee did not comply with the section cited above, the bedroom door was not maintained open while an infant napped in a crib which poses an immediate risk to the health and safety of persons in care.
