What the inspector wrote

Licensing Program Analysts (LPAs) Loraine Perez and Dao Vang met with Licensee Elshebia Ekekwe, for an unannounced case management deficiencies inspection. Licensee and one assistant were present providing care and supervision to three infants, four preschool and one school age children for a total of eight children. LPA toured the facility and observed care at the time of inspection. It was observed that one infant was napping in a room separate from the room the Licensee and assistant were in. The door to the room was not maintained open while the infant was napping. Deficiency is cited and recorded on the LIC809D. Licensee has a monitoring device in the room. Licensee immediately removed the infant from the room. LPA observed a bassinet in the living room. LPA did not observe an infant in the bassinet at the time of inspection. Licensee stated it is used for new born infants in her care. Deficiency is cited and recorded on the LIC809D. LPA reviewed Safe Sleep regulation with Licensee LPA Loraine Perez informed licensee Elshebia Ekekwe that this report dated 08/07/2025 documents one repeat Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Loraine Perez informed the licensee to provide a copy of this licensing report dated 08/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Elshebia Ekekwe .
