What the inspector wrote

Licensing Program Analyst (LPA) Loraine Perez met with Licensee Elshebia Ekekwe , regarding an Unusual Incident that took place July 2025 . LPA conducted interviews regarding the incident. Through interview it was revealed that reporting requirements were not met. Licensee did not report an unusual incident within 24 hours by phone, fax, or email. Licensee did not submit a written statement or LIC624B within 7 days to Community care licensing Child care division. In the areas that were evaluated, a deficiency was observed at the time of the visit and cited on LIC 809D. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Elshebia Ekekwe .
