What the inspector wrote

Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Kellie Rodriguez, for a case management inspection. Previously, LPA visited the facility on March 19, 2025. During the previous inspection, LPA discussed and informed the regulations of having a director or substitute director present on site. During today\342\200\231s inspection, LPA did not observe any director or assigned substitute director onsite. Per facility representative, the assigned director has been on leave since December 2024 and no information of return date. Based on the information collected, the facility is in violation of the regulation that was discussed and informed on March 19, 2025. See next page for the deficiency cited. Copy of this report was reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.
