Inspector’s narrative

What the inspector wrote

On October 8, 2025, Licensing Program Analyst (LPA) Amanda Sutter met with Infant and Toddler Coordinator Jazmyn Garrity for the purpose of an unannounced Case Management inspection. LPA observed 20 sleeping children supervised by 2 staff. On September 29, 2025, Child 1 (C1) in class Butterflies tripped and hit her head on a bookshelf. Staff 1 (S1) stated that, following nap time, C1 and Child 2 (C2) had been running in circles near the bookshelf. Immediately after C1 hit her head, S1 took the child to Coordinator who applied pressure on the wound and called 911. Staff 2 (S2) called C1's parents. Emergency Services arrived at the facility and treated the wound, and C1's parent took the child to the Emergency room. C1 received two stitches following the injury. The facility submitted a written Unusual Incident Report (UIR) on 10/1/2025. Since the incident has occurred, the facility has advised parents to provide children with grippy socks or house shoes (the facility is a shoes-off facility). LPA and Coordinator discussed other preventative measures. No deficiencies were cited in today\342\200\231s visit. Exit interview was conducted with Jazmyn Garrity and notice of site visit was posted.
