What the inspector wrote

Licensing Program Analysts (LPAs) Joshua Hatch, and Loraine Perez met with Facility Representative, Chatell Berry (Director), for the purpose of conducting a case management - deficiencies visit. The purpose of today's inspection was explained to Director During today's inspection, LPAs reviewed and obtained relevant documentation. On February 02, 2026 a exception was granted for director qualifications which expired May 8th, 2026. Director has not submitted final transcripts as stated in the exception granted. There is a citation on the following page LIC809D. In the areas that were evaluated, a deficiency was observed at the time of the visit and cited on LIC 809D . Exit interview conducted and report was reviewed with the facility representative Chatell Berry A notice of site visit was given and must remain posted for 30 days.
