Pre-licensing visit·October 1, 2021

Licensing Program Analyst (LPA) Christopher Bello arrived at the facility at approximately 12:00pm and met with applicant Valentina Ivakhov, for the purpose of a pre-licensing. Also present was applicant\342\200\231s husband. All individuals subject to criminal background review have obtained a criminal record clearance. No children were present at time of inspection. LPA began touring the facility at approximately 12:15pm. A health and safety inspection was conducted inside and out. The two-story home has a four bedrooms, three bathrooms, a living room, dining room, kitchen, garage, and fenced backyard. The off-limits areas in the home is the entire upstairs, side yard, laundry room and garage. Off-limits areas will remain inaccessible to children by closed doors and/or supervision. Toxic and hazardous items are inaccessible to children. Functioning smoke and carbon monoxide detectors and fire extinguisher were observed in the home. Current pediatric CPR was verified. The fireplace in the home was appropriately barricaded to prevent access by children. Supervision was discussed and applicant understands that children must be 100% supervised in unfenced yards. Immediate Civil Penalty regulation and deficiencies were reviewed. LPA began consulting the applicant at approximately 1:00pm consulted applicant regarding the following topics; new upcoming regulations, Safe Sleep, Type A vs. Type B deficiencies, complaints, LPA\342\200\231s right to interview children without parental consent, civil penalties, incidental medical services, placement of detergents, placement of poisons, placement of medicines, announced vs. unannounced inspections, posting requirements, unusual incident reports, On-limits vs. Off-limit areas, licensing file management, fire drills, 80% supervision, 100% supervision, fingerprint clearances, advertising, mandated reporting and best practices. LPA consulted the applicant regarding personal rights. Applicant understands that corporal, physical and/or unusual forms of punishment is never allowed toward day care children or their own children during day care hours. REPORT CONTINUES ON NEXT PAGE. LPA provided the Community Care Licensing\342\200\231s website www.ccld.ca.gov , so the licensee can obtain updated licensing information, new regulations and access forms. LPA advised licensee of their responsibility to stay current in regards to new regulations. LPA advised licensee to sign up for the quarterly updates provided by the Childcare Advocates Program. LPA provided the link https://www.cdss.ca.gov/inforesources/Community-Care-Licensing/subscribe for the licensee to sign up for the updates. This facility plans to provide Incidental Medical Services (IMS). For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Records, postings and reporting requirements were discussed. The Records To Be Maintained At The Facility - Family Child Care Home (LIC311D) was provided and discussed. Applicant was encouraged to visit the department website at WWW.CDSS.CA.GOV for information regarding child care updates, forms, self-assessment guides, regulations and legislation pertaining to family child care homes . LPA observed that a weapon that was not stored in compliance with Title 22 regulations and The Health and Safety Code. LPA observed a portion of the stairs without a gate present. LPA did not observe the facility sketch of the Upstairs portion of the home. REPORT CONTINUES ON THE NEXT PAGE. The following to be submitted prior to licensure: 1) Fire Clearance 2) Proper storage of weapon 3) Facility Sketch of Upstairs 4) Installed Stair Gate. Effective after completion of missing items, facility will be licensed for large family child care home. Facility will be approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to those for small family childcare home. An exit interview was conducted.
