Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Loraine Perez met with Licensee Vera Mikhalchuk, for the purpose of conducting an unannounced Case Management-deficiencies inspection. During today's inspection there was a census of one infant and nine preschool age children for a total of ten children in care. Present with the Licensee were her two minor children who do not count toward the capacity of the facility. The older of the two minors is an assistant to the Licensee. All individuals subject to criminal background review have obtained a criminal record clearance. The purpose of today's inspection was explained to Licensee During today's inspection, LPA reviewed relevant documentation of the facility records and children's records. Based on LPA record review, the facility had three children present who's records were incomplete and or had no records on file. Title 22 deficiencies are being cited on LIC 809D. Exit interview was conducted and report was reviewed with Licensee Vera Mikhalchuk. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
