What the inspector wrote

On 06/10/2025 License Program Analysts (LPAs) Loraine Perez met with Licensee Vera Mikhalchuk to verify correction of deficiencies that were cited on 04/28/2025. Present with Licensee is her two minor children that do not count toward capacity and one assistant. There is a census of 13 children in care. The following deficiencies previously cited are cleared at this time. * 102421(b) Based on record review Licensee has documentation for all children enrolled in care. Corrections are noted on the Cleared POC forms attached to this report. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

    A baby walker shall not be allowed on the premises of a family child care home in accordance with Health and Safety Code Section 1596.846(b) and (c).This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, Licensee used a baby walker with infants that were not able to walk on thier own, which poses a potential risk to the health and safety of the persons in care.

    INFANT SAFE SLEEP

    If an infant falls asleep before being placed in a crib or play yard, the provider shall move the infant to a crib or play yard as soon as possible. This requirement is not met as evidenced by: Based on witness statement, and interview the licensee did not comply with the section cited above, Licensee did not move an infant that fell asleep before being placed in a crib which poses an immediate risk to the health and safety of persons in care.

  • STAFFING RATIO AND CAPACITY

    For a Large FCCH, maximum number of children for whom care may be provided... when there is an assistant ...,..., shall be..:More than 12 and up to 14 children only if the criteria in Section 1597.465 of the Health and Safety Code are met. This requirement is not met as evidenced by: Based on witness statement, and interview the licensee did not comply with the section cited above, Licensee provided care for 14 children when none were enrolled in TK or above and at least age 6, which poses an immediate risk to the health and safety of persons in care
