What the inspector wrote

On 10/05/22 Licensing Program Analyst (LPA) Fabiola Diaz met with licensee Budour Khudair for an unannounced annual/1 year inspection. During the inspection there were 0 day care children present. Licensee's husband and two sons were present. All individuals subject to criminal background review have obtained a criminal record clearance. Licensee called her friend to assist with English/Arabic interpretation. The home is a a one story home with an unfenced front yard, 3 bedrooms, 2 bathrooms, kitchen, living room, garage, laundry room, and a backyard with a shed. The off-limit areas are: master bedroom, bedroom #2, garage, laundry room, shed, and sides of backyard. Licensee was notified that prior to any changes of an on-limit to an off-limit area, or vice versa, the department must be notified. No bodies of water were observed. A health and safety inspection was conducted in the areas accessible to children. The house has a working telephone, fully charged fire extinguisher and working smoke detector. LPA did not observe a carbon monoxide detector, and licensee stated she will purchase one. There are no firearms on premises. LPA observed all the required postings. LPA advised the licensee that if there are any poisons at the home, all poisons must be locked with a key lock or combination lock. LPA observed licensee's immunization records at facility file. LPA discussed the children's roster and a fire drill log. Licensee's CPR/First aid card had an expiration date of 10/02/23. Licensee understands this training must be completed every two years. Mandated Reporter Training is exempt due to language barrier. LPA observed the children's files to not contain the LIC995A form. A technical assistance note was provided. LPA provided licensee with an updated LIC311D. Report continues on 809C............... Incidental Medical Services were discussed\342\200\223 IMS. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Appeal Rights and Notice of Site Visit were provided. Notice of Site Visit must remain posted for 30 days. Exit interview was conducted and report was reviewed with the licensee. LPA Diaz informed licensee that this report dated 10/5/22 documents a Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Diaz informed the licensee to provide a copy of this licensing report dated 10/5/22 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
