Correction check·April 22, 2026

On 04/22/2026 Licensing Program Analyst (LPA) Soleil Marx met with the licensee, Dahaam Alothman, for the purpose of conducting an unannounced plan of correction inspection. During today's inspection, two day care children were present being supervised by the licensee as well as the licensee's two own minor children under the age of 10. The facility was issued two type A citations on 04/13/2026 under CCR 102417(g)(4) and CCR 102425(b) for having hazardous items accessible to day care children and for having the play yard for infant safe sleep full of articles/objects used as storage. During today's inspection LPA observed a play yard for safe sleep within the home free of loose articles and objects and observed hazardous items were stored inaccessible to children in locked cabinets/closets and high out of reach areas. LPA determined the facility has met their plan of correction; LPA has cleared the citation by visit. The facility was also cited 04/13/2026 for type B violations under CCR 102425(j)(2)(D)(c) and CCR 102417(g)(8) and CCR 102421(a) for not having children's files, a children's roster, and infant 15 minute sleep checks documented. During today's inspection, LPA reviewed records and observed children's files were complete with the required licensing forms/documentation, 15 minute sleep logs were conducted/documented for infants enrolled, and a children's roster maintained/completed. LPA determined the facility has met their plan of correction; LPA has cleared the citation by visit. LPA also observed records for licensee verifying approved CPR/first aid course was completed and is valid until 04/2028. No citations were issued as a result of today's inspection. LPA provided licensee with letters of deficiency cleared for the cleared/corrected citations. Exit interview conducted and report reviewed with Licensee. A notice of Site Visit was provided and must remain posted for 30 days.
