What Happened
Complaint investigation substantiated allegation that staff did not provide adequate supervision to children. Director failed to report incident to child's authorized representative. Type A deficiency issued for absence of supervision with $500 immediate civil penalty.

(1) No child(ren) shall be left without the supervision of a teacher at any time...shall include visual observation. This requirement was not met as evidenced by:based on observation, interview, and record reviews, Director(D) did not comply with the section cited above as a child was left unattended in the play yard for approximately 11 minutes. D stated that staff failed to conduct their attendance counts while transitioning from outside to inside, which poses an immediate health, safety, or personal rights risk to persons in care.
