01/29/26 at approximately 9:15 AM LPA Andrea Cortez arrived at Excelsior Preschool and was greeted by Director Heather Robertson. LPA explained to nature of the visit was to conduct a Case Management – Incident Inspection to follow-up on an unusual incident report (UIR); received in the Sacramento Regional Office on 1/27/26. LPA set up in the Directors office. LPA requested rosters for both teachers and children, other supporting documentation. LPA and Director toured the classroom and playground areas. LPA inspected the classroom where the reported incident occurred and took photos. LPA observed the teacher to children ratio 1:7. LPA observed children playing on playground, two children were sitting at a table with a broad game while others running and interacting. Based on LPA interview with teacher and Director further investigation into the UIR is warranted. No Title 22 Regulations cited. Exit interview conducted
