What the inspector wrote

Application Specialist (AS) Mai Lor and Licensing Program Analyst (LPA) Alize Tillery met with Applicant Kathleen Breeding for the purpose of an announced pre-licensing change of ownership inspection. This is a change of ownership from Carousel School (#340312678). Applicant is requesting a preschool license to serve 73 preschool children from age two years to entry into Kindergarten. Census included 14 preschool children and 3 staff. The program will operate Monday through Friday from 7:30 AM to 5:30 PM. The fire clearance was granted on 04/29/2022. AS reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. Applicant stated the facility will provide AM/PM snack, and a lunch. INDOOR ACTIVITY SPACE: There are five preschool classrooms: Classroom #1, #2a, #2b, #3, #5. AS and LPAs observed sufficient amount of age- appropriate equipment, toys, tables, and cubbies. There is a kitchen to support the food service equipped with a stove and refrigerator. The Applicant was requested to ensure that the refrigerators are in good repair. Applicant stated she will be replacing the refrigerators. There is a first aid kit in the office. Applicant stated medications will stored in the office. Applicant stated there are no firearms or weapon on the premises. Applicant stated indoor and outdoor drinking water is provided via water pitchers with individualized water bottles. AS and LPA observed stained walls and carpets throughout the facility. Applicant stated she will be replacing the carpets throughout the facility and paint the building. AS and LPA observed a carbon monoxide detector in the facility. AS and LPA observed paper sign-in/sign-out system. (Report continued on subsequent LIC 809) AS and LPA measured the five classrooms. The total classroom space contains a total of 3326.92 square feet, which will accommodate the requested capacity for 73 preschool children. There is a total of 8 toilets and 7 sinks operating and available for children to use in the facility. One sink is located in the infant classroom. There are three non-operating sinks and two non-operating toilets in addition to the operating sinks and toilets. AS and LPA observed the sinks and toilets stained and rusted. Applicant stated she intends on replacing the non-operating sinks and toilets. AS and LPA observed some light fixtures throughout the facility needing repair. Applicant stated light fixtures will be fixed or replaced. There is a separate private restroom for the staff. Children who become ill during the day will be isolated in the office and will use the staff restroom, if necessary. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). OUTDOOR ACTIVITY SPACE: There is an outdoor activity space for preschool. The outdoor play area is surrounded by a fence that is at least four feet tall. AS and LPA observed large spacing between the fencing and the ground. Applicant stated she planned on replacing the fence; however, in the meantime, she will put a temporary fence to fix the spacing. The preschool yard is separate from the infant play yard. The preschool playground has a play structure that is anchored to the ground. AS and LPA did not observe a safety label. The play structure appeared to be designed for older children. AS and LPA recommended Applicant to find the appropriate safety label to ensure that the play structure is appropriate for the ages that the facility is providing services for. AS and LPA observed woodchips used as cushioned under climbing structure. Applicant stated she planned on replacing the play structure designed for ages 3-5 years old. She stated that she will replace the woodchips with poured in place rubber. AS and LPA did not observe sufficient amount of age-appropriate equipment and toys. There are shaded areas supplied by trees and overhangs. AS and LPA measured the outdoor space. The outdoor play area contains a total of 9198.33 square feet, which will accommodate Applicant's request for 73 preschool children. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). AS discussed the following: 100% supervision is required at all times; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. AS discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days. (Report continued on subsequent LIC 809) This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. Applicant was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for child care updates, current forms, legislation and regulation information. A copy of this report will remain on file for a period of three years for public review upon request. Carpets need to be washed and cleaned 3. Fencing gap shall be enclosed 4. Napping cots to be assembled 5. Light fixture shall be fixed to ensure ample lighting in each of the classrooms 6.
