On Friday, November 21, 2025, Licensing Program Analyst (LPA) Lea Habtom arrived at the facility for an unannounced case management inspection. LPA met with the designated representative, Priyanka Sharma , due to the absence of the director Sithumini Millawabandara. LPA provided the purpose of the inspection is to follow up on the plan of correction for the deficiencies cited on October 16, 2025. There were 17 preschool children being supervised by 3 staff. All staff present during today's inspection have fingerprint clearances and associations. The director arrived during the inspection and completed the inspection with LPA. LPA reviewed staff files and confirmed 1 staff member completed the mandated reporter training. There were 2 staff members with expired mandated reporter trainings. LPA reviewed children files and confirmed that the children files are complete with the necessary forms. This report was reviewed with the director, Sithumini Millawabandara. Civil penalty was assessed during today's inspection. A notice of site visit was provided to be posted for 30 days. Appeal rights provided.
