On January 9th, 2024, Licensing Program Analysts (LPA) Pa Dao Vang met with Director Bianca Miraflores for a case-management inspection for an unusual incident occurred on 12/16/2023. Upon arrival 26 preschool children napping were present supervised by 3 staff members. The facility self-reported an incident by submitted a written Unusual Incident Report LIC624 on 12/20/2023, regarding personal rights issue. LPA made observations and conducted interviews during the inspection. Due to the prepond erance of the evidence and context of the situation, LPA determined no Title 22 violation in regards to the report. Exit interview conducted and report was reviewed with Director Bianca Miraflores . Additionally, a copy of this report, Notice of Site Visit, and appeal rights were provided to Director.
