On February 26, 2026, Licensing Program Analysts (LPAs) Joshua Hatch and Amanda Sutter met with Licensee Maria Jimenez for the purpose of a Plan of Correction (POC) inspection. On February 10, 2026, two type B citations were issued. Plans of Correction (POCs) were originally due 2/24/2026. Upon arrival, LPAs observed 3 children at the facility, including one infant under 12 months old. LPAs observed licensee holding the infant and the two remaining children playing in the living room supervised by licensee. LPAs requested to view the backyard area to observe if the type B deficiency for a thorn bush in an on-limits area accessible to children was corrected. LPAs observed a thorn bush that was trimmed back and barricaded making it inaccessible to children thereby correcting the type B deficiency. LPAs inquired if the licensee had completed mandate reporter training to correct the type B deficiency for not completing mandated reporter training. Licensee stated that mandated reporter training was still not completed and that it would be completed by tomorrow 2/27/2026. While in the backyard area LPAs observed cigarette butts on the ground in an on-limits area accessible to children. PAGE 1. REPORT CONTINUES ON LIC809-C Based on the inspection, one Title 22 Deficiency has been issued on the attached LIC 809-D. The licensee was informed that this report dated 2/26/2026 documents one Type A citation which shall be posted for 30 consecutive days. The licensee shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A civil penalty of $200 has been assessed for failure to correct deficiencies cited. Licensee has been provided with appeal rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Maria Jimenez.
