What the inspector wrote

Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Director Katelynn Lee (Director), for the purpose of conducting an unannounced complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained. During the investigation, LPA conducted interviews, observed care, and obtained and reviewed relevant documentation. LPA reviewed facility attendance documents and did not observe any documented instances of infants and preschool children being commingled. On 04/14/2025 and 06/02/2025, LPA observed care and did not observe infant and preschool children commingled. LPA did observe a toddler option class, which is licensed to provide care to children 18-36 months of age. LPA interviewed children, staff, and parents. Witness statements failed to corroborate the allegation of commingled infant and preschool children.Witness statements, LPA observations, and document reviews failed to corroborate the allegation. Although the allegation may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Katelynn Lee. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
