101416.5 Staff-Infant Ratio (b) There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met as evidenced by: LPA arrived at facility at 7:55 AM and observed census of 7 infants and 2 staff; at 8:00 AM, 8th infant was in attendance, and at 8:12 AM, 9th infant was accepted. Staff messaged the other facility for help, but no one was available to step in until LPA observed Director walked in at the facility at 8:17 AM which poses an immediate health, safety, or personal rights risk to persons in care.
