What the inspector wrote
Licensing Program Analyst (LPA) Erwina Pascual-Golamco met with Applicant Vijaya Lakshmi Koppula, for the purpose of a pre-licensing inspection. There was one cleared adult and no children in the home during the inspection. All individuals subject to criminal background review have obtained a criminal record clearance. Applicant plans to operate Monday through Friday from 8:30 AM to 5:30 PM.
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) was discussed with Applicant. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. A health and safety inspection were conducted inside and out. This facility is a two story home. The first floor has a bedroom, bathroom, dining room, living room, kitchen with walk in pantry, and garage. The second floor has 3 bedrooms, 2 bathrooms, laundry room and loft.
The off-limit areas will be: the entire second floor, living room, pantry in kitchen, garage, left and right side of yard.
Off-limits areas will remain inaccessible to children by closed doors and/or supervision. LPA did not observe a safety gate to barricade the upstairs area. Applicant understands that whenever children under the age of five are present the safety gate must be up, preventing access to the second floor. Toxic and hazardous items are inaccessible to children and are stored in the kitchen pantry's top shelf. Functioning smoke and carbon monoxide detectors and 2A-10-BC fire extinguisher were observed in the home. LPA received EMSA certificate for Preventative Health, Safety, Nutrition, and Lead Poisoning Class training, Applicant's Mandated Reporter Training (MRT) is valid until
08/2025
and EMSA Pediatric CPR and First Aid training is valid until
07/2025
. Applicant understands both CPR and MRT must be renewed every two years. Applicant stated she understands that her own children under the age of 10 count as part of the ratio. Applicant stated that there are no weapons in the home. LPA did not observed a fireplace in the home. The backyard is fenced, and supervision was discussed and applicant understands that children must be 100% supervised in unfenced yards. Immediate Civil Penalty regulation and deficiencies were reviewed. There are no bodies of water on the premises.
LPA consulted applicant regarding the following topics; new upcoming regulations, Type A vs. Type B deficiencies, complaints, LPA\342\200\231s right to interview children without parental consent, civil penalties, incidental medical services, placement of detergents, placement of poisons, placement of medicines, announced vs. unannounced inspections, posting requirements, unusual incident reports, On-limits vs. Off-limit areas, licensing file management, fire drills, 80% supervision, 100% supervision, fingerprint clearances, advertising, mandated reporting and best practices.
LPA consulted the applicant regarding personal rights. Applicant understands that corporal, physical and/or unusual forms of punishment is never allowed toward day care children or their own children during day care hours. LPA provided the Community Care Licensing website

, so the Applicant can obtain updated licensing information, new regulations, and access forms. LPA advised licensee of their responsibility to stay current regarding new regulations.
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPA reviewed with applicant the LIC311D, Forms/Records to Keep in Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Applicant was encouraged to visit the department website at

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. LPA advised Applicant to sign up for the quarterly updates provided by the Childcare Advocates Program. On this date, 08/07/23, the California Attorney General - Megan's Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
The following items are required before a license will be issued:

At the time that LPA receives documentation that the above items are completed, this small family childcare home will be licensed.
