On 10/16/2025 Licensing Program Analyst (LPA) Andrea Cortez met with Director Vanessa Garcia for an unannounced Case Management Inspection regarding an Unusual Incident Report (UIR) reported to the Sacramento Regional Office on 10/10/2025. LPA observed 36 children supervised by 5 staff members. UIR describes a child running on the playground when the child tripped and fell on their left arm resulting in calling 9-1-1. The 9-1-1 representative dispatched Fire Safety to examine the child\342\200\231s arm and placed a splint to secure the arm. The ambulance arrived and transported the child and a family member to the hospital where child was diagnosed with a broken arm. During today\342\200\231s inspection LPA observed the area where the injury occurred,took photos, obtained a signed incident report, roster, and interviewed the Director that reported the incident. LPA determined that further investigation is required to assess the removing any tripping hazards. An exit interview was conducted, and the report was reviewed with Director Vanessa Garcia A Notice of Site Visit was provided and shall remain posted for 30 days.
