What the inspector wrote

Title 22 Deficiencies have been cited on the attached LIC 9099D. Facility Representative was informed that this report dated 03/19/2025 documen ts two Type A citations which shall be posted for 30 consecutive days. Upon receipt of Type A citations, the facility shall provide copies of the LIC 9099D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signe d LIC 9224, Acknowledging Receipt of Licensing Reports LIC 9099D in each child's files. Exit interview conducted and report was reviewed with Facility Representative Jennifer Parsons. A notice of site was provided and must remain posted for 30 days. Appeal rights provided.
