What the inspector wrote

Based on LPA's observations and interviews, which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the allegation is found to be SUBSTANTIATED. Licensee will be assessed a citation A. Licensee will have currently enrolled families sign the LIC 9224 "Acknowledgment of receipt of licensing reports," as well as all new/incoming families, for a year from this date of January 24, 2024. The LIC 9224 must be placed in each child's file. Failure to do so, will result in a subsequent citation. This report must be available for all families to review. An exit interview was conducted and a notice of site visit was provided, which must be posted for 30-days. Deficiency is recorded on 809-D
