What the inspector wrote

On Tuesday, May 19, 2026, Licensing Program Analyst (LPA) Tanya Washington conducted an unannounced case management inspection and met with Licensee Amber Alton. Upon arrival, LPA observed the Licensee providing care and supervision to four daycare children ages 1, 3, 3, and 4. During today's inspection, LPA conducted a file review for Staff #1 and observed the following information missing: - Mandated Reporter Training certificate - Tuberculosis clearance - Proof of Measles vaccine - Influenza vaccine or a statement from Staff #1 declining the vaccine. Staff #1 provided care and supervision to children at the facility the week of May 11, 2026 though May 15, 2026. The Licensee was advised to ensure that all required documents for Staff #1 are obtained and maintained in the personnel file by the Plan of Correction date of 06/19/2026 or prior to Staff #1 return to assist. The Licensee may provide proof of the missing information to LPA via email. Type B violations are cited on the following LIC 809D. Notice of Site Visit was provided and appeal rights were explained.
