On Tuesday, 30 September, 2025, at approximately 10:00am, Licensing Program Analyst (LPA) Fabian Schwartz met with Licensee\342\200\231s Spouse, Karan Hanzraa, for the purpose of an unannounced annual inspection. Upon arrival, LPA observed Licensee\342\200\231s Spouse supervising 5 preschool children, 3 of which are infants. Licensee was not present for duration of inspection, but arrived toward end of inspection. Licensee\342\200\231s Spouse informed LPA that the home had a new resident which did not have a fingerprint clearance with Department\342\200\231s Guardian system. New Resident is named Sandeep Shaina Kaur and she moved into home on 25 September 2025. LPA observed proper ratio and capacity was being followed. Facility hours of operation are Monday-Friday from 8am-5pm. LPA verified that the annual fees are current. A health and safety evaluation was conducted in all areas accessible to children. Facility is a 5 bedroom, 3 bathroom, two story house. Off-limit areas include: Entire Upstairs and Garage. Licensee acknowledged that children may never enter these off-limit areas. LPA observed that the facility is clean, sanitary, and in good repair. LPA observed a functioning smoke detector, carbon monoxide detector, and a full 2A10BC fire extinguisher. LPA observed that there is a fenced backyard area with no pool. LPA reviewed 4 children\342\200\231s files which LPA observed to be complete, however, one child in care did not have any paperwork at facility. LPA observed infant sleep plan (LIC 9227) and 15-minute observation checks for napping infants. Required postings were seen in front entryway, and the children\342\200\231s roster was observed. The facility has record of conducting fire drills at least every 6 months, with the last drill being conducted on 4 September 2025 . Licensee\342\200\231s CPR/First Aid card expires on 30 December 2025 . Licensee\342\200\231s Spouse did not have a valid CPR certificate while supervising children alone. Licensee\342\200\231s Mandated Reporter training certificate expires on 27 February 2026. Licensee\342\200\231s spouse has a valid Mandated Reporter Certificate. Licensee understands that trainings must be completed every two years. PAGE 1. REPORT CONTINUES ON LIC809-C\342\200\246\342\200\246. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. A notice of site visit was given and must remain posted for 30 days. Based on the inspection, 2 Type A and 2 Type B Title 22 Deficiency Citations have been issued. Type A citations are for Licensee not being present in facility while childcare is operating and for new home resident not having a fingerprint clearance. Type B citations are for not having a child\342\200\231s file and for Assistant not having CPR certificate while being alone supervising children. The Type A Citation for Unfingerprinted adult comes with a Civil Penalty of $500. Citations are explained further on LIC809-D pages. Title 22 deficiencies are cited on the subsequent pages of this report. Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Licensee. LIC 9224 and Appeal Rights were provided. Licensee's signature on this report acknowledges receipt of these rights. Exit interview conducted and report was reviewed with Licensee. A notice of site visit was given and must remain posted for 30 days.
