What the inspector wrote

On 3/27/2025 at approximately 10:50AM , Licensing Program Analyst (LPA) Michelle Perez met with Licensee , Polina Kashirskaia, for an unannounced inspection. During the inspection there was a census of 4 CHILDREN (3 preschool & 1 infant) supervised by the licensees . All individuals subject to criminal background review have obtained a criminal record clearance. Facilities hours of operation are Hours Monday through Friday 7AM to 5:30PM. A health and safety inspection was conducted in the areas accessible to children. The off-limit areas Bedroom #2 and #3. hall bathroom, shed and garage. Licensee understands that children may never enter these off-limits areas. The house has a working telephone, fully charged fire extinguisher, smoke detector and carbon monoxide detector that meet regulations. LPA observed all required postings. LPA observed home was safe, orderly, and free of hazards. LPA advised the licensee that if there are any poisons at the home, all poisons must be locked with a key lock or combination lock. LPA observed a fireplace within the home and it is barricaded. The licensee stated that there are no firearms or bodies of water on the premises. LPA observed a children's roster and fire drill log, the last fire drill was conducted February 2025. Licensee's have current CPR/First aid,one of which expires August 2025 . Licensee\342\200\231s Mandated Reporter Training expires March 2027 & June 2026 . Licensee understands both training\342\200\231s must be completed every two years. LPAs reviewed records of children\342\200\231s files. Missing documents include LIC 9224, from a previous type A citation, Parent Rights that are not completed and medical consent forms that are not completed. Incidental Medical Services (IMS) policy was discussed. No children on medication. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Licensee was not practicing safe sleep regulations, and did not keep an infant sleep log.
