Follow-up on corrections·May 21, 2026

On Thursday, May 21, 2026, Licensing Program Analyst (LPA) Lea Habtom arrived at the facility for an unannounced case management inspection. LPA met with licensee's two assistant's. There were 7 children consisting of 3 infants and 4 preschool children. The licensee arrived during the inspection. All staff present during today's inspection have fingerprint clearance and associations. LPA arrived at the facility to open a complaint and a case management inspection was created to address the infant safe sleep deficiencies. There were 3 infants sleeping in cribs with bottles and blankets. One infant was wrapped in a blanket. Based on LPAs observation, a Type A deficiency was cited for loose articles in the crib. LPA Lea Habtom informed licensee Dedrionna Lewis that this report dated May 21, 2026, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the licensee Dedrionna Lewis to provide a copy of this licensing report dated May 21, 2026, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
