On Tuesday, May 20 th , 2025 , Licensing Program Analyst (LPA) Payenda Seddiqi met with Licensee, Anastasiia Kontsemal for the purpose of an announced change of location inspection. Licensee was previously licensed under facility # 343625451. During today\342\200\231s inspection, licensee, licensee's spouse and mother were present in the home. The visit was conducted in English language. All individuals subject to criminal background review have cleared background checks. Licensee plans to operate Monday through Friday from 8:00 am to 7:00 am (24 hours). A health and safety inspection was conducted inside and outside the home. The home consists of 4 bedrooms, 3 bathrooms, a kitchen, 1 family room, 1 living room and a backyard. The off limits areas include bedroom 1, bedroom 2, bedroom 3, and bedroom 4, family room, and bathroom 2 and bathroom 3. Licensee understands that children may never access these off limit areas. There is an operational heating stove in the corner of kitchen in the living room. Licensee stated that it is used only once a while and she will have supervision over it. A fire clearance was granted 05/16/2025. Toxic and hazardous items are inaccessible to children and out of children\342\200\231s reach. LPA advised the licensee that if there are ever any poisons at the home, all poisons must be locked with a key lock or combination lock. A functioning smoke detector and carbon monoxide detector was observed. LPA observed a fully charged 3A40BC fire extinguisher within the home. Licensee states there are no weapons in the home. Licensee has completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training. Current EMSA approved pediatric CPR/First Aid trainings were verified and expires 09/2025. LPA discussed the requirement to renew CPR/First Aid training every two years. Licensee has also completed her Child Abuse & Neglect Mandated Reporter Training which will expire 08/31/2025. LPA provided the website mandatedreporterca.com. Licensee was encouraged to maintain supervision at all times. Licensee understands that if an unusual incident occurs, licensing is to be notified via phone call, e-mail or fax within 24 hours and the Unusual Incident Report LIC 624 shall be submitted within 7 days to remain in compliance. LPA discussed Type A/B citations, open door policy, 100% supervision in unfenced areas and near bodies of water, fire drills, children\342\200\231s personal rights, and smoking prohibition with the licensee. A current roster of children enrolled must be available and maintained for a period of three years, even after children are no longer in care. LPA reviewed ratio and capacity. Licensee understands that children that live in the home under the age of 10 years old count towards total capacity. LPA explained to licensee that if they relocate and wants to continue to provide care, they must submit a change of location application and have the new home inspected. Licensee understands that if they want to make any off-limit area an on-limits area, licensing must be notified, and LPA must do an inspection before children are allowed in the area. Licensee understands that licenses are not transferable, and once licensed, licensee must live in the home and be present for 80% of the operating hours. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The licensee provided proof of control of property. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Child Care Center (CCC) and Family Child Care Home (FCCH) licensees are responsible for ensuring that each child\342\200\231s needs can be met at the time of admission and throughout a child\342\200\231s attendance at the facility. Children with disabilities or special health care needs may require medications while in care. Childcare staff can learn to safely give medications to children, and these services are known as IMS. LPA reviewed with licensee the LIC 311D, Forms/Records to Keep in Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the licensee. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. On this date, 03/13/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California Exit interview conducted and report was reviewed with the licensee Anastasiia Kontsemal. LPA discussed CCR Regulation 102416.5 regarding staffing Ratio and Capacity for a large license. LPA discussed personnel requirements and records outlined in CCR Regulations 102416 and 102416.1. Licensee understands that when an assistant is not present, she must comply with the capacity and ratio requirements for a Small Family Child Care Home. Effective today, 05/20/2025, the facility is approved for a large capacity to serve 12 children (when there is an assistant present) with no more than 4 infants or capacity of 14 children when 1 child in kindergarten or elementary school and 1 child at least age 6 and a maximum of 3 infants. Infants are children under the age of two years.
