On 6/2/2026, Licensing Program Analysts (LPAs) Amanda Sutter and Mandie Goodwin met with Director Ines Chekkat for the purpose of an unannounced Case Management inspection. LPA observed 30 children supervised by 3 staff. At 1:30 PM, LPAs observed children in the large classroom and adjacent nap room. Three staff were present in the classroom. Children were either on nap mats or standing in the large classroom. Staff sent multiple children into the lobby to put on their shoes. These children then left through the back door toward the playground with Staff 1 (S1). Staff 3 (S3) walked toward the lobby to supervise children. LPAs observed Staff 2 (S2) leave the large classroom and walk into staff restroom in the facility lobby. S3 then walked outside with S1. At this point, no teachers were left to supervise the children in the nap room adjacent to the large classroom. LPAs walked to the nap room adjacent to the restroom and observed 9 children sleeping on mats. No staff were in the classroom. S2 walked into the classroom at 1:33 PM, while S1 stayed in the room adjacent to the nap room and S3 stayed outside with the awake children. Based on the inspection, one Title 22 Deficiency has been issued on the attached LIC 809-D. The director was informed that this report dated 6/2/2026 documents one Type A citation which shall be posted for 30 consecutive days. The director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. Because the citation involved an absence of supervision, an immediate civil penalty of $500 has been issued. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Director Ines Chekkat. A notice of site visit was given and must remain posted for 30 days. Director has been provided with appeal rights.
