Pre-licensing visit·May 15, 2025

On Thursday, May 15, 2025, at approximately 11:30 AM, Licensing Program Analyst (LPA) Josiah Gathing met with Applicant Mohammad Sedeq Nikbeen for the purpose of an announced pre-licensing inspection. Applicant plans to operate Monday \342\200\223 Sunday 24 hours. All persons subject to criminal record background review have been cleared. A health and safety inspection was conducted inside and outside the home. The single-story home has a master bed/bathroom, two additional bedrooms, an additional bathroom, two living rooms, a kitchen, a garage, and a back yard. Off-limits areas: Bedroom #2 and Garage. LPA observed a functioning smoke detector, carbon monoxide detector, 2A10BC fire extinguisher, and first aid kit in the home. Per applicant, there are no weapons or poisons in the home. Applicant completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training on February 25, 2025. Applicant\342\200\231s EMSA certified CPR and First Aid Certificate expires August 12, 2026. Applicant\342\200\231s Mandated Reporter Certificate expires March 22, 2027. Applicant was encouraged to maintain supervision at all times in unfenced areas. Type A/B citations and Immediate Civil Penalty regulation deficiencies were reviewed. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Entrance Checklist was provided to the applicant. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. On this date, April 16, 2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. As of today May 15, 2025, facility is approved for a Small Family Child Care Home license for a capacity of 6 children with no more than 3 infants, or 4 infants only; or up to 8 children with no more than 2 infants, with 1 child in Transitional Kindergarten or above and 1 child at least age 6. Infants are children under the age of 2.
