What the inspector wrote

On Tuesday, August 12, 2025, Licensing Program Analysts (LPAs) Tanya Washington and Julia Maryanova met with Applicant, Olena Stasyuk for a second pre-licensing/ change of location inspection. Upon arrival LPAs conducted a tour of the entire home inside and out. There were no children or other adults in the home during today's inspection. Applicant advised LPAs that she has decided to make changes to the areas accessible to children. During today's inspection, Applicant stated that she wishes to add the master bedroom/ bathroom which is located at the end of the hallway past the kitchen area. The bathroom closest to the kitchen area will be off limits to children. LPAs toured the sauna/ hot tub room and observed newly installed straps with four latches that are locked. Applicant unlocked the latches during the inspection to show that the locks are operational. LPAs gained access to one motor home and two travel trailers located in the back of the property. The motor home and both trailers appeared to be unoccupied. Per Applicant, the motor home and two travel trailers do not belong to her and belong to the owner of the property. Applicant was made aware that she must have access to all areas on the property and if someone occupies the trailers or motor home they must be fingerprint cleared. Updated off limit areas of the home consist of: entire 2nd floor, first bedroom/ bathroom on the right side of the home which has a separate entrance from the main home, sauna/ hot tub room, living room/ bedroom attached to the sauna room, gazebo to the left of the home in the driveway near the gate and back field area past the fenced children's outdoor playground. Applicant acknowledged that off limit areas must remain inaccessible to children. Applicant was advised that she would need to contact the department before making any on-limits areas to off-limits or vice versa. Continued on LIC809C During today's inspection, LPAs observed that the playground area has been cemented, LPAs observed that artificial grass has been installed and there are age appropriate playground equipment accessible to children. LPAs also toured the the barns and observed that locks are installed. LPAs also observed that all tools and construction equipment has been removed and there are no hazards accessible to children. Applicant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Effective today 08/12/2025, this facility is licensed for large family child care home. Facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to those for small family childcare home. Exit interview conducted and report was reviewed with the applicant Olena Stasyuk.
