On August 12, 2025, Licensing Program Analyst (LPA) Soleil Marx met with Licensee, Liza Nezami, for the purpose of conducting an announced pre-licensing change of location inspection from previous license #

. All individuals subject to criminal background review have obtained a criminal record clearance. Hours of operation will be Monday through Friday from 8:00 AM to 10:00 PM.
Licensee provided proof of control of property. The house is a 3 bedroom 2.5 bath, two story home. Off-limit areas include: entire second floor and garage. Off-limit areas are made inaccessible by locked/closed doors, baby gates, and supervision.
A health and safety inspection was conducted on the interior and exterior of the home. Toxic and hazardous items such as sharp knives, cleaning solutions, and medications are inaccessible to children in high cabinets and child locked cabinets/drawers. LPA observed a functioning smoke detector, carbon monoxide detector, and a fully charged 2A10BC fire extinguisher within the home. LPA observed electric fireplace within the home is barricaded by screen and glass. LPA observed stairs to upstairs are barricaded by baby gate. LPA observed toys and equipment available to children within the home. Licensee stated that poisons are stored in a key locked cabinet within the off-limits garage; LPA reminded licensee that if there are ever any poisons at the home, all poisons must be locked with a key lock or combination lock. Licensee stated there are no firearms/weapons on the premises. Licensee has current EMSA approved pediatric CPR/First Aid training expiring 11/2025 and current Mandated Reporter Training expiring 08/2026. Licensee was reminded that both CPR and mandated reporter training must be completed every two years.
Licensee has an above ground spa/hot tub in the enclosed backyard. LPA observed that the spa/hot tub is inaccessible to children in care by having a hard top cover in good-repair that can withstand the weight of an adult and the cover is key locked on all four corners.
The California Attorney General - Megan\342\200\231s Law website was searched on 07/30/2025 for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
LPA reviewed with licensee the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, postings, capacity, supervision, reporting requirements, type A/B citations, open door policy, personal rights.
LPA provided consultation on safe sleep environments, infant sleep plans, infant safe sleep checks, and infant regulations. LPA discussed the safe sleep regulations with

LPA informed

of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
LPA reviewed with applicant infant devices that are not allowed in a Family Child Care Home.
Effective today August 12, 2025, This facility is approved for their change of location of Small Family Child Care Home license for a capacity of 6 children with no more than 3 infants, or 4 infants only, or up to 8 children with no more than 2 infants, 1 child in Transitional Kindergarten or above and 1 child at least age 6. Infants are children under the age of 2.
Exit interview conducted and report reviewed with Licensee. Because this is a change of location of a previously licensed facility, a notice of site visit was given to licensee and must remain posted for 30 days.
