Inspector’s narrative

What the inspector wrote

(1) Staff did not adequately supervise infants in care, resulting in injuries. It was alleged that staff did not adequately supervise children in care, resulting in injury to one infant in care after another infant in care made physical contact. Based on observation during facility tours, LPAs observed staff to maintain ratios in the infant classroom and observed staff providing direct visual supervision of children in care. Based on interviews, statements provided offered conflicting accounts regarding the position of staff at the time of the alleged incident and the duration of the alleged incident. LPA Hatch reviewed facility video footage covering the time of the alleged incident and documents pertaining to the incident which did not did not corroborate the allegation. Based on the evidence available although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. (2) Staff handled an infant in a rough manner. It was alleged that staff handled an infant in care in a rough manner during a transition between positions. Based on observation during facility tours LPAs observed staff to use under-the-arm support when picking up infants in care and did not observe any instance of rough physical handling of children in care. Based on interview, staff stated that the appropriate method for picking up a child in care is under-the-arm support. Based on interviews, statements provided offered conflicting accounts regarding the allegation and did not corroborate the allegation. Based on the evidence available although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Page 2. Continued on LIC9099-C... (3) Staff spoke inappropriately in the presence of children. It was alleged that staff at the facility spoke about a child in care in an inappropriate manner in the child's presence. Based on observation during facility tours LPAs did not observe staff speaking inappropriately about children in care or speaking inappropriately in the presence of children in care. Based on interviews, statements provided offered conflicting accounts and did not corroborate the allegation. Based on the evidence available although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. A Notice of Site Visit was given and must remain posted for 30 consecutive days. Exit interview conducted and report was reviewed with facility representative Aaliyah Harris. Appeal rights were provided to facility representative Aaliyah Harris.
