What the inspector wrote

LPA provided Licensee Appeal Rights to D1. A Notice of Site Visit was posted by LPA and shall remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100.

    BUILDINGS AND GROUNDS

    101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement was not met as evidenced by: A broken front door that LPA observed on 04/17/2023. Children attempted to exit and adults were able to enter without permission.This is a potential health and safety risk to children in care.

    Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... This requirement was not met as evidenced by: Interviews revealed that staff held a child down to a chair and staff grabbed a child by the arm to hold down on a nap mat. This was a health and safety risk to the children in care.

    Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance…This requirement was not met as evidenced by: Interviews revealed that the facilty was operating out of ratio in the afternoons. More than 12 children were under the care of 1 teacher. Aides were supervising children in the capacity of a qualified teacher. This posed a health and safety risk to children in care.
