On 10/17/24, Licensing Program Analyst (LPA) Corina Beckby conducted an unannounced annual inspection. LPA met with Licensee, Fereshteh Alizadeh. LPA observed Licensee caring for 1 child (4 years old). LIC (126), Entrance Checklist for Family Child Care Homes, was provided and reviewed with Licensee. Facility hours of operation are Monday through Friday 7:00 am to 7:00 pm. Licensee has also applied to change from a Small Family Child Care Home to a Large Family Child Care Home. Fire Safety Inspection Clearance was granted on 09/24/24 by Cosumnes Community Services District FD, for a total capacity of 14 children. All individuals subject to criminal background review have obtained a criminal record clearance. LPA verified the annual fees are current. Licensee stated there are no new residents in the home. All adults subject to criminal background review have obtained criminal record clearance. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. A health and safety inspection was conducted in all areas accessible to children. Home appears orderly and suitable for children. T he OFF-limits areas are the second floor, garage, downstairs bedroom, pantry and shed. Off limit areas are being made inaccessible by closed doors, gates and supervision. Licensee acknowledged that children may never enter these off-limit areas. Hazardous items were inaccessible to children. Napping equipment and age-appropriate toys were observed. LPA observed the required postings, a working telephone, a 2A10BC fire extinguisher, and functioning smoke and carbon monoxide detectors. The backyard is fenced. Licensee understands that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes. LPA toured the kitchen area and verified knives were inaccessible to children in care. LPA observed playroom areas with age-appropriate toys for children. Licensee is responsible for ensuring all child proof locks and gates are secured and in working order. 7 Children's files were reviewed. Emergency information and required immunization records were on file. LPA observed a current roster and documentation that a fire drill is conducted at least once every six months. Licensee's immunization records are available in the facility file. Current EMSA pediatric CPR and First Aid certification was verified and expires on 04/29/25. Licensee has current Mandated Reporter Training Certificate that expires 11/20/25 and understands Mandated Reporter Training must be completed every two years starting January 1, 2018, and retain proof of completion in the facility file. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA discussed the requirement to check and log infant napping every 15 minutes for infants under 24 months. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA discussed and explained new ratios for large license. Licensee was reminded to contact the department of any facility closures. Licensee does not have children requiring IMS. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for childcare updates, current forms, legislation and regulation information. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, Licensee, Fereshteh Alizadeh, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. An Exit interview was conducted, and the report was reviewed with Licensee, Fereshteh Alizadeh. LPA posted a notice of site visit. Licensee understands the Notice must remain posted for 30 days and that a failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were provided. A copy of this report will remain on file for a period of three years for public review upon request. The licensee's signature on this form acknowledges receipt of this form. Effective today 10/17/2024, facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to a small family childcare home.
