On 11/04/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a field visit to the facility for the purpose of an unannounced annual inspection. LPA disclosed the purpose of the inspection and was granted entrance into the facility. LPA met with the Director Deborah Ochoa (D1). LPA toured the facility inside and outside. LPA observed 6 infants being supervised by 2 staff members. LPA accessed Guardian to verify that all required adults were background cleared and associated to the license. LPA observed 1 volunteer providing care and supervision to children who did not have a cleared background check. D1 confirmed that there are no Registered Sex Offenders employed at the facility. LPA completed the RSO profile on 10/10/2025. Facility days and hours of operation are Monday-Friday from 7:00 AM to 5:30 PM. The facility does not provide transportation. LPA conducted a file review prior to inspection and notified D1 that annual fees were not current. While on location, LPA observed that hazardous items (disinfectants, cleaning solutions etc.) were inaccessible to children in care. LPA reviewed care and supervision of children, staffing ratios, medications, first aid supplies, furniture, equipment, fire drills, and drinking water. LPA observed all required forms to be posted. Cribs were available for each infant under 12 months old and cots available for infants over 12 months old. A Needs and Services Plan was on file for each infant/toddler present during today\342\200\231s inspection. LPA observed proper storage of refrigerated bottles which were dated and labeled with children's names. LPA discussed the safe sleep regulations with D1 and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. CONTINUED ON 809-C LPA discussed the Infant Sleep Regulations with D1. LPA discussed the requirement to check and log infant napping every 15 minutes and observed Napping Logs for infants 24 months and under. LPA observed Individual Sleeping Plan/LIC 9227 for infants during today's inspection. LPA also informed D1 of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended that s/he register all infant devices with the CPSC to be notified of any recalls on purchased equipment. LPA observed cribs with pads being used as mattresses with sheets that were not properly fitted. LPA observed a changing table within arm's reach of the sink. There appeared to be adequate toys and equipment available for children. LPA toured the outdoor play area with available shade. D1 stated that a pitcher of water is taken outside for recess. LPA reviewed the paper sign in/out documents and observed that the children who were currently in care were signed-in. LPA reviewed children\342\200\231s and staff files. LPA observed Health Screening Reports with TB test and required MMR and TDAP vaccines for some members present today. A staff member present today had current Pediatric CPR and First Aid training (expiration 10/2026). LPA observed current Mandated Reporter training for some staff members present today. D1 was reminded to renew the course every 2 years. This training requirement may be met by using the Department\342\200\231s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com . The training is currently provided in English and Spanish. This facility provides Incidental Medical Services (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers constructed before January 1, 2010, CONTINUED ON 809-C to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP on November 2023. D1 was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, PRIOR to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. D1 was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Title 22 deficiencies were cited on subsequent 809-D page(s). LPA informed Licensee Ochoa that this report, dated 11/04/2025, document(s) 2 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Velazquez informed L1 to provide a copy of this licensing report dated 11/04/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, and the report was reviewed with Director Ochoa. LPA provided Licensee Appeal Rights to D1. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
