On 12/10/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a Case Management inspection to verify corrections of a deficiency cited on 11/04/2025. LPA arrived at the facility and was met by Director Deborah Ochoa (D1). LPA disclosed the purpose of the inspection and was granted entrance into the center. LPA toured the facility and observed 38 preschoolers being supervised D1 and 3 staff members during nap time. LPA determined, through accessing Guardian, that all required adults were background cleared and associated to the license. On 11/04/2025, the facility was cited a Type B deficiency for not having complete children's files including immunizations. During today's inspection, LPA reviewed children's files and observed the required forms and immunizations. No deficiencies were cited during today's inspection on 12/10/2025 in the areas that were evaluated. LPA cleared the citation and provided D1 with a letter of clearance. An exit interview was conducted, and the report was reviewed with Director Ochoa. Licensee Appeal Rights were provided to D1. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Failure to comply with posting requirements will result in an immediate civil penalty of $100.
