What the inspector wrote

On 03/03/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a field visit to the facility for the purpose of a case management inspection regarding a self reported Unusual Incident Report (UIR) dated 02/20/2025. LPA arrived at the facility and was met by Director Lexi Simmons (D1). LPA disclosed the purpose of the inspection and was granted entrance into the facility. LPA toured the facility and observed 8 preschool aged children being supervised by 2 staff members. LPA determined through accessing Guardian that all required adults were background cleared. LPA collected and reviewed documentation. LPA requested a copy of the Facility Roster. LPA conducted staff and children interviews. Interviews and information collected supported what was reported in the UIR: a child (C1) made a disclosure to their parent(s) that a staff member (S1) had inappropriate contact with the child. The staff member has been removed from the facility, the incident was reported, and the incident is currently being investigated. No deficiencies were cited in the areas that were evaluated today on 03/03/2025. An exit interview was conducted, and the report was reviewed with Director Simmons. LPA provided D1 with Licensee Appeal Rights. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
