What the inspector wrote

On 06/21/2024, Licensing Program Analyst (LPA) Corina Beckby met with Licensee, Metra Ayoubi, for the purpose of an unannounced annual inspection. Licensee was present supervising 2 of her own children (ages 4 months and 2 yrs). LIC (126), Entrance Checklist for Family Child Care Homes, was provided and reviewed with Licensee. Facility hours of operation are Monday through Friday 8:00 am to 5:00pm. Licensee has also applied to change from a Small Family Child Care Home to a Large Family Child Care Home. Fire clearance was granted by Cosumnes Community Services District FD on 06/05/2024. LPA verified the annual fees are current. Licensee stated there are no new residents in the home. All adults subject to criminal background review have obtained criminal record clearance. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. A health and safety inspection was conducted in all areas accessible to children. Off-limits areas include: second floor, garage, backyard & front yard. Off limit areas are being made inaccessible by closed doors, gates and supervision. Licensee Report continues on LIC809-C\342\200\246 page 2... acknowledged that children may never enter these off-limit areas. LPA observed the required postings and a working phone and 2A10BC fire extinguisher that is serviced yearly. LPA observed functioning dual smoke and carbon monoxide detector. LPA toured the kitchen area and verified knives were inaccessible to children in care. Licensee stated there are no weapons in the home. LPA observed age-appropriate toys for children. Licensee is responsible for ensuring all child proof locks and gates are secured and in working order. Licensee understands that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes. LPA reviewed with Licensee, ratio regulations for a small and large family child care home. LPA reviewed 6 children's files. Emergency information and required immunization records were on file. LPA observed a current roster and documentation that a fire drill is conducted at least once every six months. Licensee's immunization records are available in the facility file. Current EMSA pediatric CPR and First Aid certification was verified and expires on 12/03/24. Licensee has current Mandated Reporter Training Certificate which expires 11/29/24 and understands Mandated Reporter Training must be completed every two years starting January 1, 2018 and retain proof of completion in the facility file. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA discussed the requirement to check and log infant napping every 15 minutes for infants under 24 months. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was encouraged to visit the Department website at WWW.CCLD.CA.GOV for childcare updates, current forms, legislation and regulation information. Licensee was reminded to notify the department of any and all closures (holiday, illness, vacation, other). Continued on LIC809-C... page 4... Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, Licensee, Metra Ayoubi, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Effective today 06/21/2024, facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to a small family childcare home. An Exit interview was conducted, and the report was reviewed with Licensee, Metra Ayoubi. LPA posted a notice of site visit. Licensee understands the Notice must remain posted for 30 days and that a failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were provided. A copy of this report will remain on file for a period of three years for public review upon request. The licensee's signature on this form acknowledges receipt of this form.
