Facility-requested visit·October 30, 2025

On 10/30/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted a Licensee initiated Case Management inspection for the purpose of a room addition. LPA arrived at the facility and was later met by Director Megan Kirwin (D1). LPA disclosed the purpose of the inspection and was granted entrance into the facility by the Elementary School staff. A Fire Clearance was issued on 10/16/2025 and is on file with the Department. The current hours of operation for the facility are Monday thru Friday 2:45 pm until 6:00 pm due to low enrollment for a before school program. INDOOR ACTIVITY SPACE: The program is adding 1 room, A3, to the license. Currently, the program utilizes rooms A19 and the multipurpose room for after-school care. LPA inspected the room A3 and observed 1 sink. LPA observed the facility has a total of 3 toilets, 1 urinal, and 4 sinks available. This is sufficient to meet the needs of 60 children. The facility\342\200\231s capacity is 60 school aged children. Licensee is not making a change to the licensed capacity. LPA measured Room A3 and the room contains a total measurement of 488 square feet. Individual measurements are recorded on the Capacity Worksheet LIC 9024. LPA is approving the use of classroom A3 as of today, 10/30/2025. No deficiencies were cited today in the areas that were evaluated. An exit interview was conducted, and the report was reviewed with Director Kirwin. Licensee Appeal Rights were provided to D1. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
