Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

PERSONAL RIGHTS
To be accorded dignity in his/her personal relationships with staff and other persons. This requirement was not met as evidenced by Since 10/12/2023, child 1 has bitten four times and one attempted bite. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION
No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by Staff failed to see child bite on 10/30/2023 resulting in injury and only observed the child who was bit crying and teeth marks on child's hand.This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

Each licensed child day care facility shall post a copy of any licensing report pertaining to the facility that documents either a facility visit or a complaint investigation that results in a citation for a violation that, if not corrected, will create a direct and immediate risk to the health, safety, or personal rights of children in care. The licensing report provided by the department shall be posted immediately upon receipt, adjacent to the postings required pursuant to Section 1596.817 and on, or immediately adjacent to, the interior side of the main door to the facility and shall remain posted for 30 consecutive days. This requirement was not met as evidenced by LPA did not observe required posting for Type A violations issued on 10/12/2023. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

ARTICLE 2. Administration of Child Day Care Licensing
A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b This requirement was not met as evidenced by Sarah stated she had reports available but did not give to parents but informed them and had them sign LIC9224. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.
