Staff member with missing TB test documentation. Supervision breach with school-age child entering bathroom alone without staff awareness.
Observations
- Staff 3 does not have valid TB test on file
- One school-age child observed entering bathroom without supervision
- Staff unaware child entered bathroom alone

Based on observation, the licensee did not comply with the section cited above. LPA observed one school age child go into the bathroom without supervision. Staff were unaware child entered bathroom alone which poses an immediate health, safety or personal rights risk to persons in care.
