What the inspector wrote

On the date and time listed, Licensing Program Analyst (LPA) Eric Ramos arrived at the facility for the purposes of conducting a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) that occured on 10/1/2025. LPA was granted entry into the facility by Melissa Williamson. LPA explained the purpose of today's inspection, completed a facility tour, conducted interviews and obtained a census. On 10/02/2025, it was reported that a staff member slapped a child's hand. Through interviews conducted, only one staff member stated they saw the incident occur, however, there was no further information obtained to corroborate the allegation. Based on information obtained during the investigation, it has been determined there is no violations of Title 22 Regulations pertaining to the reported incident. No deficiency cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Melissa Williamson.
